Indiana University School of Medicine and Health Focus, Inc. is committed to honoring your privacy, and to offering special protections for the personal information you share with us.

Information We Collect
In this section of our Privacy Policy, we discuss the different types of information we may collect about you, and the ways in which we collect this information.

Information We Collect From Visitors
Visitors to our websites can browse the website without disclosing any personally identifiable information. We may track information provided to us by your browser, including the website you came from (known as the "referring URL"), the type of browser you use, the time and date of access, and other information that does not personally identify you.

Information We Collect When You Register
Users registering for a CME or CE activity on the web site are asked to provide to us identifying information, such as name, email address, and other identifying information. On our registration screen, we clearly label which information is required for registration, and which information is optional and may be given at your discretion. We use the personal information that you provide through registration to respond to questions and email from you, and to contact you if we have problems or questions.

Information We Collect When You Contact A Thought Leader Online or Via Email
When you submit a question to a thought leader by email or on one of our web sites, we receive your email address and/or name. We share this identifying personal information with the thought leader so that he or she can send an email reply directly to you.

Information We Collect When You Complete A Posttest, Evaluation, or Survey
When you complete a CME or CE posttest, evaluation, or survey form online, we collect the information we need to issue a CME or CE certificate to you. We also compile this information in aggregate form, and may provide it to third parties with all identifying information removed. Commercial supporters of CME and CE activities on our web sites receive only aggregated data about CME or CE activities that are relevant to their interest and/or the courses they have supported.

Use of Cookies
Cookies are a technology we use to identify a user as the user moves through one of our web sites. Your browser allows us to place some information on your computer's hard drive that identifies the computer you are using. We use cookies to personalize our web sites and to track your usage on them.

Children's Privacy
Our web sites are not designed to attract children. Anyone under the age of 18 should not use our web sites. We do not collect any personal information from children under 18 years of age. We try to comply fully with the laws and regulations relating to collection and use of information from children, including the Children's Online Privacy Protection Act of the United States.

How We Use Information
In this section of our Privacy Policy, we identify the ways we may use information about you that we have collected.

Aggregate Data
We create aggregate data about visitors to our web sites for web site development and to improve aspects of our web sites such as ease of use, navigation, etc. We also create aggregate data using data provided to us in CME or CE posttests, evaluation, and survey forms. We may provide this information in aggregate form to third parties, but will do so only with identifying information removed.

How We Protect Information About You
In this section of our Privacy Policy, we discuss the security measures we take to protect information that we have collected about you.

General Policies
We have implemented technology and security policies, rules and other measures to protect the personal data that we have under our control from unauthorized access, improper use, alteration, unlawful or accidental destruction, and accidental loss. We also protect your information by requiring all our employees and others who have access to, or are associated with, the processing of your data to respect your confidentiality.

Storage of Personal or Professional Information
Any personally identifiable information you share with us is stored on our database server at the Health Focus, Inc. data center or hosted by third parties who have entered into agreements with us that require them to observe our Privacy Policy. We have a firewall to prevent individuals from accessing information without authorization. The data center is physically secure and protected from unauthorized access by unauthorized persons.

Information in our data center is backed up routinely in order to aid in the recovery of information in the event of accidental damage of information or due to a natural disaster. The backup media is stored in a physically secure storage facility.

Updating Your Personal Information
You can review or change your registration information online on our web sites. You also can review online on our web sites your personal CE or CME activity history or histories. Finally, you can have your files permanently deleted from our systems by sending an email to us instructing us to permanently delete your records. Your request should be emailed to mpurdy@healthfocusinc.com.

Changes to Our Privacy Policy
If we change our Privacy Policy, including any of the policies or practices described here, we will post the changes online and clearly identify them on a revised copy of this document. To draw attention to these changes, we will put a prominent notice on the front page of this web site that says, "Important Changes to Our Privacy Policy. Click Here."

Areas Not Covered by Our Privacy Policy
This Privacy Policy applies only to this web site owned by Indiana University School of Medicine and Health Focus, Inc. This web site may contain links to other web sites that we do not own. When you leave this web site and enter a web site not owned by us, we are not responsible for the privacy practices of that web site.

Our Privacy Policy does not protect you when you send business communication, ideas, concepts or inventions to us by email or over the Internet. If you want to keep business information, ideas, concepts or inventions private or proprietary, do not send them via email or over the Internet.
